Shaping Our Lives is a non-profit, user-led organisation working for the inclusive involvement of Disabled people and people from other seldom heard groups. They are looking to involve people with lived experience in shaping their service design and delivery to ensure that their services are inclusive and accessible.
The My Involvement Profile was designed by Disabled people and based on our research where deaf and Disabled people told us that they don’t always record their support requirements, and don’t always have their access requirements met.
The Profile is a simple, free template form, which comes in a variety of formats including Easy Read, which participants can use to record access and support requirements, and their involvement experiences and interests.
My Involvement Profile has two parts:
- A personal information form to record your contact details and other experience you have
- A form to record your access and support requirements.

For local VCSOs striving to meet access needs but also struggling with lack of funding and increased demand, the My Involvement Profile could help them to collate access needs in one simple way. The Profile can also be used by people who want to share their lived experience to help make local and national services better, something many of your members will be actively involved in or campaigning for.
