Organisation description: Outreach Community and Residential Services Outreach has for almost 40 years been successfully providing a range of services supporting adults with a learning disability and/or long term mental health needs, to live an independent life. Our aim is to provide person centred support to people living either in their own homes or within our residential services.
Outreach was formed in 1978 after a group of parents who had children with a learning disability, from within the Jewish community, wanted a more independent life and support within the community for their children.
We have developed a range of services including intensive support for people with challenging behaviours, dual diagnosis, autism specific services, services for people with additional physical and sensory needs and community support for people who are more independent. We work closely with families, carers and other stakeholders to provide co-ordinated services for individuals. There is a high level of active service user participation with regular forums for service users with learning disabilities.
What we do
We offer a range of services which includes registered residential care homes with up to a maximum of 5 beds, supported living accommodation (shared and self contained flats) and domiciliary support in the community. We also facilitate leisure groups and a drop in centre.
We work with adults with a wide spectrum of needs and can tailor individual packages of support to meet their specific needs.

The Outreach Community & Residential Services ethos is to ensure that service users are empowered to fully identify and express their personal goals and provided with the support to achieve them by developing their individual person centred plan and monitoring progress.
