The Salford Healthy Communities Collaborative Programme (SHCC) is a 2-year programme which is designed to build on the previous SHCC pilot which operated in three wards in Salford.
The aims of the SHCC are,
• Address health inequalities in areas of socio-economic disadvantage
• Be the catalyst enabling communities and agencies to work together for common goals
• Harness the skills and knowledge in communities and make them work to reduce inequalities
• Recognise and change poorly functioning systems using rapid improvement techniques.
We will achieve these aims by focusing on the two biggest killers of Salfordians, Cardio Vascular Disease (CVD) and Early Detection of Cancer and using this focus to address community specific issues that we can have an outcome on. At the end of two years we have made visible changes to the services and facilities for people in the area and we will be able to demonstrate that our actions have increased the use of health checks and have prompted more people to go to the doctor early enough to have their suspected cancer checked and if needed, treated.
