Job Summary

To co-ordinate the creative use of existing social care support to help realise the social and leisure aspirations of adults living in Warrington who have a learning disability.

To work collaboratively with existing projects and provider organisations to maximise access to all aspects of community life with the aim of:

  • facilitating the development of friendships and relationships
  • reducing social isolation
  • Improving access to new social opportunities and activities
  • Increasing confidence to make and maintain friendships

Key Tasks

  • Research the opportunities available for individuals to access community groups/activities and events (in particular in the evenings and at weekends)
  • Collate this information in the most appropriate format i.e. newsletter, website etc.
  • Raise awareness of the opportunities available for individuals to access community groups/activities and events
  • Research and establish demand from individuals to access community groups/activities and events
  • Initiate and co-ordinate communication between provider organisations (who have committed to supporting the project) to facilitate the sharing of commissioned support hours
  • Work collaboratively with partners to meet the needs of individuals where there is no appropriate opportunity available
  • Establish and record the impact of the project for the purpose of demonstrating how effective it has been in achieving its aims
  • Any other reasonable request within the capability of the successful candidate
